Support Coordination
We provide support coordination to help you understand your plan and what supports and services you can choose from to achieve your goals and be able to live independently. Our Support Coordinators will assist in:
• Finding the right service provider for you, whether it be Allied Health, Service care supports, or Capacity building for your skills.
• Help break down your plan and assist with connecting you with the suited funded supports to maximise your plan.
• Coordinate meetings with yourself, the NDIA, and service providers to manage your progress and outcomes
• Arrange assessments based on your needs to assist in quality of life
• Training Carers and support workers to ensure the appropriate skills are utilized to help achieve your goals
Plan Management
We offer dedicated Plan Management services to help clients make the most of their NDIS plans. Our team assists with understanding your funding, connecting you with the right service providers, and ensuring your supports are managed effectively and transparently.
Home Modifications
